I would suspect the system was not adequately vacuumed before the initial charging, contaminated refrigerant or lubricant was installed, or the rec/drier was already contaminated w/ moisture when installed.

If it's never been opened since initially charged and no leakdowns, there is no path for external moisture to enter the system. Internal pressure always > external pressure.
