sounds like you are doing everything right...
Another thing, on a GM VAN with long lines, under the
vehicle lines, etc.. you may want to very carefully
visually check all the lines (metal) for exterior corrosion
from road salt, etc, as it is a known failure mode also...
and spray with "tar" (Z-bart) or some heavy compound to keep
oxygen/salt/water off the metal lines under the vehicle... this can
be done without opening the system up though.
Cooltop charge is .93 X the R134a charge by weight, calc
with REFPROP and practice seems to bear it out as correct.
